Your Details On Sunday 10 November 2019 at 11.00am, the annual Remembrance Day Service will take place at Holy Trinity Church in Fareham to Forever Remember those who gave their lives in the service of their country.

If you would like the name of a loved one who died whilst serving in a British Armed Forces conflict, to be added to a list of names to be read out at the Memorial, Holy Trinity Church from 9.45am, please complete the form by 25 October.

Please note that if the name of your loved one already appears on a memorial within the Borough of Fareham, you do not need to complete this form, as their name will already be on the list to be read out at the special service on 10 November.

Please note, for the purpose of Data Protection: The personal information you have provided will only be used for the purpose of Equality Impact Assessment in Fareham Borough Council and for auditing, monitoring, statistical and other research.
